    Psychiatric Evaluation and Medication Management Services

    Medication Management Follow-Up: $150 for 30 minute appointment

    Initial Psychiatric Evaluation: $300 for 90 minute appointment

    Services Not Covered by Insurance

    Disability and FMLA Reports /Forms Completion: $75.00/15ā€

    Telephonic Evaluation (self-pay only per 15ā€): $75.00/15ā€

    Pharmacogenetic Test Administration: $25

    Copies of Medical Records (other than continuity of care): $50.00

    Late Cancellation/Missed Appointment: $150 for initial evaluation and $75 for Medication Management Follow-Up

    Declined Payment Fee: $25

    Court or Legal Related Services: Please contact office for fees

    Insurance

    Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for mental health medication management services.

    Iā€™d recommend asking these questions to your insurance provider to help determine your benefits:

    • Does my health insurance plan include mental health benefits?
    • Do I have a deductible? If so, what is it and have I met it yet?
    • Do I have a copay for my appointments? If so, what is it?
    • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
    • Do I need written approval from my primary care physician in order for services to be covered?

    Payment

    I accept cash and all major credit cards as forms of payment. I also accept payment directly via the patient portal.

    Cancellation Policy

    If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Otherwise, you may be charged for a no show fee. If you are more than 10 minutes late to a follow-up appointment you may be charged a no show fee.
